Born In Quebec:
- Birth Certificate (Large Format)
- Last transcript of school marks (Report Card)
Born outside Quebec but in Canada:
- Birth Certificate
- Last transcript of school marks (Report Card)
- Hydro Quebec Bill or Lease (if not in your name bring a letter from person whose
name is on the bill or lease, stating that you live with them)
Born Outside of Canada:
- Birth Certificate (from country of birth)
- Documents stating status in Canada
(Landed Immigration Form, Canadian Citizenship Card, Permanent Residency Card)
- Refugee Claimant (Situation Statutaire, CSQ and Notice of Decision)
- Last transcript of school marks (Report Card) or Equivalency
- Hydro Quebec Bill or Lease (if not in your name bring a letter from person whose
name is on the bill or lease, stating that you live with them)
